The result is malfunctioning service, which aborts or block almost everything.
However, its not completely clear how to for example run c-icap service. In example its set as user squid and group squid.
Or should I use SuSE defaults and run as root?

You should modify the steps in your guide as follows…
Instead of building from source, search for and install packages from openSUSE if they are available instead.
You can inspect the systemd Unit files if they exist to verify location of configuration files (or read MAN pages) and modify if needed.
Modify start/stop services and applications using systemd commands instead when available(which will likely be installed as part of the packages from openSUSE)
