When do you want said program to start really? It would help if we knew what program in order to advice when it should be started.
If when the Desktop starts just put a script/or shortcut in ~/.kde4/Autostart
To add a program to be autostarted select the menu / Personnel Settings / Advanced Tab / Autostart. You can enter the program name or the name of the script. Be aware that not all programs go by their name you use for them. For instance the System Monitor is called ksysguard, not that you would autostart that, but it pays to check the name using the menu editor, searching for the program you want to start when you log into KDE. I even wrote a script, to write a script, to put it in the KDE4 autostart folder. You only need to know the name of the file to autostart.

When auto starting things, think about your normal start-up in KDE. Go to menu / Personnel Settings / Advanced Tab / Session Manager. If you leave a program running when you log out of KDE, it starts up again when you log back in. If you autostart the same program AND it allows itself to be loaded more than once, you can end up with two of them loaded. I normally select to start with an empty session, but be aware of this operation in KDE.

Good replies from both those guys for local programs.
But if it’s a program that needs to be started by root, you can address it in the file boot.local which sits in directory /etc/init.d
Or if it needs to wait for runlevel 5, use the file after.local
also knowing just WHAT it is would help
for example i run ISIS3 from nasa/usgs and need to set some system settings at boot
for that i use a startup script . That i put in /etc/profile.d
so without knowing what it is you need to do …
