When you apply a patch, the path code is merged with the code on the system (or the product is downloaded completely) and the downloaded patch is deleted (normaly). Only the resulting patched product is there. So there is no ‘old patch sitting somewhere’. A newer patch contains (logicaly) also the code of any patch you did not apply earlier.
So there is no leftover and no worry.
This is a ‘short’ explenation, but I think it covers enough to give you an answer.

I can not tell you how this works in Win 98 or XP, never used them. But what you tell me gives me the shivers :X
openSUSE will never do this, that is correct.
