I am currently enjoying the current stable KDE 4.5.0 on openSUSE 11.3 thanks to the Index of /repositories/KDE:/Distro:/Factory/openSUSE_11.3 repo. However, I realized that when KDE 4.5.1 development starts, or maybe when 4.6.0 development starts, that this repo will also provide those dev version, correct? So how can I keep up to date with the current STABLE release of KDE on openSUSE 11.3 without using dev releases?

You need to not update KDE then. You see AFAIK there will be no maintained kde4.5 repo.
There is no problem doing that. Just set your system to install only security patches. That will hold your system as it is.
So let’s suppose that I install openSUSE a few months from now and I want to update to the latest stable KDE 4.5.2 or whatever is current at that time. It appears that version will never be available, but rather some dev version? I currently am lucky to have upgraded my KDE immediately after 4.5.0 was released, so I currently have it on my system. But what about a new install that wasn’t timed like that?
That’s good to hear that the Factory has been relatively stable for you. I know that openSUSE 11.3 will always officially use KDE 4.4.4, but I guess what I was looking for was something like a “KDE Backports” repo, something similar to the Kubuntu PPA that offers all the 4.x.y point releases for the currently stable Kubuntu.
So at what point will the …/Factory/openSUSE_11.3/ repo stop providing 4.5.x releases and switch to 4.6-dev releases?
