I finally yielded. NM just was not going to admit to a leased IPv6. Instead I went manual on it.
# nmcli connection modify eth0 ipv6.method manual ipv6.addresses 'fd2f:4760:521f:3f3c::c0a8:456d' ipv6.gateway 'fd2f:4760:521f:3f3c::c0a8:4501' ipv6.dns 'fd2f:4760:521f:3f3c::c0a8:456d'
