How to develop forms using SAMBA File share

HI There
I am really thankful to the people on this forum, now my file server is running without any problems. and I really admit that its easier and fast than Windows Server.
Well i am planning to make the forms system in openSUSE instead off Windows in my organization. If someone has complain/request so they can fill out the the Form, from any computer and concern department can check it out in their departmental computer.

So what type of services and components i need for this purpose. i’ll deeply appreciate your help

The easiest way for making/handling request forms would be PHP. Your suse server is already up & running, configure your LAMP server!
