App icons disappear/error symbol when app is closed, fine when open?

Hi All

Some of the icons of pinned apps are appearing in the taskbar as the x-errors session file symbol (Black X on a grey background) when the app is closed, but the correct app icon appears when I open the app. It goes back to the error icon after I close the app again.

Any ideas?

Which session type do you use, KDE/Plasma? Gnome? XFCE? LXDE? Mate? Cinnamon? Something else?
