After connecting/disconnecting once to an AP, the networkmanager applet fails to list the AP until networkmanager is restarted. Other APs do appear in list.
The AP is in B/G mode, auto channel selection, WPA/WPA2 PSK. This works fine with 12.1, 11.4
Does it get refreshed if you open a terminal and do something like
su -c 'iw eth1 scan'
(* using the network device relevant to your system of course)
I’ve had the same issue on occasion with openSUSE 11.4, KDE4.8, whereby my iPhone (configured as a wireless AP) would not be detected by my laptop, until I did this.
displays my AP on cmd line but the nm still doesn’t see it so I have to restart network service. In the past this happened with a KDE update so its probably a KDE (NM) regression bug.

Possibly, in which case it should be reported. However, it is worth checking that your connection is defined as a ‘System connection’, and that the WPA2 passphrase is recorded correctly. Read this thread:
The psk is correctly saved in the settings, which is why I can connect when it sees it.
If it is made system connection, I have to enter root pwd to connect every time.
Another problem is that I am sharing my home dir with previous version of opensuse also updated to kde 4.8.4. The NM passphrases are stored in kwallet. However, logging in to 12.2, I could not see these settings and kwallet’s kded crashed on start, until I cleared networkmanagementrrc.
I confirm the same behavior with my OpenSuse12.2 64bit, KDE 4.9.1. I can list my AP with “iwlist scan”, but still it isnt listed in the network applet.
The error is solved if I unable, then re-enable the wifi network.
Below are the final lines from my /var/log/NetworkManager when the error occurs:
